Completed PRC forms and the required accompanying documents must be apostilled by your Secretary of State.

For California residents and practitioners, in-person apostille services are available in Sacramento and Los Angeles. Mail-in apostille is also available. For details, click here.

For non-California residents and practitioners, please contact your state's office for general apostille services.

Apostille services will be available on-site during the orientation scheduled for October 18.

Application Process

Fill out application online

Verify your email. You will receive a link to fill out an online application. Follow instructions on the form and submit your application and required documentation.

All volunteers will need to upload a copy of their passport information page.
Submit required documents

If you are a licensed medical volunteer, a committee representative will contact you if you need to submit additional documentation. It is important that we receive the necessary documents by August 31, 2025*.

Once we receive all the necessary information and documentation, you will receive a formal approval email confirming your spot on the medical mission team.
Attend an orientation

Plan to attend the orientation and any meeting with the team you will be assigned to.
