Passport Page

All foreigners (non-Filipino citizens) are required to submit a copy of their Passport Information page. This is required by the Philippine Consulate and the local Philippine government where the medical mission will take place.

Taking a photo of your passport page requires attention to detail and ensuring that all necessary information is clearly visible while protecting your privacy. Here's a step-by-step guide:

  1. Prepare a well-lit area: Find a location with good lighting to ensure that the text on your passport page is clear and readable. Natural daylight is often the best option, but if that's not available, use indoor lighting without harsh shadows.
  2. Place your passport on a flat surface: Lay your passport open on a flat surface such as a table or desk. Make sure the page you're photographing is fully visible and not obstructed by anything.
  3. Position your camera or smartphone: Use a camera or smartphone with a high-resolution camera to capture clear details. Position the device directly above the passport page, ensuring that it's parallel to the page to avoid distortion.
  4. Focus and zoom: Adjust the focus of your camera to ensure that the text and details on the passport page are sharp and clear. Avoid using digital zoom, as it can reduce image quality. Instead, physically move the camera closer if you need to capture specific details.
  5. Check for glare and reflections: Be mindful of any glare or reflections on the passport page, especially if there's glossy laminate covering the information. Adjust the angle of your camera or lighting to minimize glare and ensure that all text is visible.
  6. Capture the entire page: Frame the photo so that the entire passport page is visible within the viewfinder or screen of your camera. Make sure none of the edges are cropped out, as this could omit important information.
  7. Review the photo: After taking the photo, review it to ensure that all details on the passport page are clear and legible. Check for any blurriness, glare, or obstructions that may affect the quality of the image.
  8. Retake if necessary: If the photo is not satisfactory, retake it until you're satisfied with the clarity and quality. It's essential to have a clear image of your passport page.

Professional License

Some volunteer positions require a license. Refer to the table below to find out what documents you need to submit depending on the choice of volunteer assignment. If you do not have a valid license during the medical mission, consider other positions that do not require any documents other than a valid passport.

PRC License

If you indicated that you have a PRC License, then you will be asked to upload a copy. PRC-licensed volunteers pay a smaller fee because they do not require additional administrative costs such as apostille and notary services, etc. associated with filing of the required PRC Form.
